Do I have to be a United States citizen to use the Evolution Rx Card?
No you do not have to be a United States citizen to use the Evolution Rx Card
How soon can I start using the plan and saving?
Immediately!
Are there waiting periods, deductibles, co pays or preexisting condition limitations?
No. Since your Evolution Rx Savings Plan is not an insurance plan, there are no waiting periods, deductibles, co-pays or pre-existing condition limitations. There also are no forms to complete or submit. And you don't need to wait to be reimbursed. Just show your card at the time of service to receive your discount.
Can other members of my household use my Evolution Rx Savings Plan? Yes. Anyone who resides at your address can automatically receive savings with your plan.
How does this plan work with my existing Plan?
You can also apply this plan to services not covered by your medical insurance. In many cases, certain prescription drugs and vitamins and nutritional supplements are not covered by medical insurance plans.
Does the card work in Canada or on-line pharmacies?
No at this time he card is only accepted in the United States and online pharmacies are presently not accepted.
Will I receive the same savings every time I refill or get a new prescription?
No, your discount may fluctuate as each pharmacy has the right to change their prices on a daily basis, thus adjusting the amount of your savings.
Does every store in a chain recognize this card?
Yes, our contracts are for the entire chain of locations.
Can my cousin or sister use the card I purchased?
Yes, they sure can! As long as they share the same family last name.
My pharmacy is not a nationwide chain and I wish to move my business to receive the benefit of this card, how do I do that?
It is simple. You can go to any participating pharmacy and either present your refill bottle or take the actual prescription to the location. You may also elect to have your current pharmacist contact the pharmacy with the prescription information.
What do I do when I receive my card?
Once you receive your card detach the bottom portion of the card and complete the perforated card with your name and any 9 digit number. You can then immediately use the card at any Participating pharmacy.
